Before Your First Visit

Complete New Patient Form(s)​

After completing the proper forms, please email them to [email protected] as an attachment or print and bring to your first visit.
Send Us Any Recent Radiographs (X-Rays)

If you don’t have them, reach out to the office that conducted the X-Rays and ask them to email them to [email protected].

Please Share All Dental Records With Us

Any periodontists, orthodontists, or general dentists that you’ve seen should be keeping detailed records of your care.

Have them forward those records to [email protected].

New patients are always Accepted

It is important to arrive at our office at least 15 minutes before your appointment so we can prepare all necessary forms and records and address any concerns you may have.

You don’t need to change your eating habits before your first visit. During the 90-minute consultation, we’ll review your history, perform a clinical assessment, and recommend a treatment plan.

If you haven’t emailed your forms and records to our office, please bring a printed copy to your appointment.

At Your Appointment We Will...

You will consult with one of our skilled dentists, who will carefully examine your forms, dental records, and X-rays submitted before your appointment.

We will discuss your specific needs and concerns to understand your ultimate goals.

We’ll examine your teeth carefully, inspecting each surface, and the structural integrity of each tooth, your bite, your lips, and facial contours. You will be screened for tooth decay, oral cancer, and gum (periodontal) disease.

Out of Town Patients’ Guide

  1. Schedule a consultation appointment.
  2. Fill out forms and send over the necessary records.
  3. Travel to our office for a consultation visit, where we will create plans for the next steps.
  4. Schedule treatment appointment.
  5. Return to office 1-2 days later for the treatment to receive temporary teeth.
  6. Return to the office at least 3 months after the initial treatment to begin the permanent teeth process.

On average, we recommend staying for 3-5 days.
